By submitting this application form, I/we declare that I/we have read and agree to be bound by the Team Food Island Program Requirements. I/we understand that we are considered enrolled in the Initiative upon receipt of written approval from a Team Food Island program partner.

I/we will be required to furnish the names and contact information of employees hired as part of the Team Food Island Student Job Bonus program.
I/we will also be required to retain and to furnish, on demand by Team Food Island, the PEI Seafood Processors Association, SkillsPEI or the Office of the Auditor General of Prince Edward Island all proofs relating to the employment of, and payments of bonuses to, student workers participating in the Team Food Island program.
